AIDS

  • Multitude of signs and symptoms

Acanthosis nigricans

  • Benign hyperpigmentations in groin, neck, genitalia

Bechets disease

  • Ulceration of genitals, mouth,
  • Iritis, scleritis, optic neuritis, vessel sheathing, retinal edema, glaucoma, cataracts
  • More common in Japan
  • Treated with steroids

Cicatricial pemphigoid

  • Apparent chronic conjunctivitis
  • Slowly progressive cicatrization of the conjunctiva and oral mucosa

Dermatomyositis

  • Skin rash, proximal muscle weakness, difficult walking, nasal soundind voice, aspiration, papules on fingers
  • Periorbital edema and erythema (Heliotrope)

Erythema multiforme

  • Blister formation
  • Mild form attacks only hands and feet
  • Severe form is Steven-Johnson syndrome
  • Young men in spring or fall for 2-3 weeks
  • Fever, malaise, headache, urinary infection,
  • Bullous red lesion with clear center on extremities and mucous
  • Conjunctivitis, sicca, corneal ulcer
  • Skin lesions and erosion of mucous membranes
  • May be asociated with meds
    • Sulfonamides
    • Pennisulin
    • Aspirin
    • Phenylbutazone
    • Barbiturates
    • Scopolamine
    • Sulfonamides
    • May follow H. Simplex, measles, vaccination, radiation

Lupus

  • Erythematous rash exacerbated by sunlight (Butterfly rash is classic sign), oral and nasal ulcers
  • KCS,SPK,conjunctivitis, episcleritis
  • Cotton wool,retinal hemes, retinal occlusion, optic neuropathy, uveitis

Pemphigus

  • Blistering of skin and crusting
  • Erosion of eyelid margins and trichiasis
  • Conjunctiva slightly injected with slight discharge

 

Ployartertis nodosa

  • Vasculitis of small and medium-size blood vessels
  • Skin nodules following the course of superficial arteries
  • Retinal vasculitis, CNS changes

Psoriasis

  • Red plaques and silvery scales on elbows, knees, ears
  • Nail dystrophy
  • Conjunctivitis, corneal vascularization and opacities

Reiter's syndrome

  • Arthritis, conjunctivitis, urethritis
  • Circinate balanitis (Penile vesicles)
  • HLA-B27

Relapsing polychondritis

  • Auricular chondritis (swelling, erythema, tenderness in the external ear)
  • "Saddle nose"
  • KSC, scleritis, corneal infiltrates

Rheumatoid Arthritis

  • Rheumatoid nodules, Vasculitis lesions, pyoderma gangrenosum (Necrotic skin ulcer)
  • Sjogren's syndrome, keratitis, scleritis

Sarcoid

  • Hilar adenopathy,Erythema nodosum, plaques, nodules, scarring
  • Koeppe and Busacca nodules, iritis, venous occlusions
  • Granulomas of lid, conjunctiva, lacrimal gland, choroid and optic nerve
  • Retinal periphlebitis with "candle wax" drippings

Scleroderma

  • Tight skin,thin pused lips, dull expression
  • KCS, cotton wool

Seborrheic blepharitis

  • Results in increased meibomian secretions
  • Chronic long lasting
  • More sever in winter
  • Stress and illness exacerbate
  • May be auto-immune or may be related to Pityrosporum ovale yeast
  • Foamy tears from excess oil
  • Greasy scales ("Scurf") that itch surrounded with some erythema
  • Dandruff does not have erythema, only scales
  • Most common site is scalp, but also seen on ears, eyebrows and chest.
  • Eczema is a term used in children when the diagnosis is not sure to be atopic dermatitis or seborrheic dermatitis

Vogt-Koyanagi-Harada

  • Alopecia, vitiligo, poliosis
  • Uveitis, choroiditis, optic neuritis
